Manuscript, written in black ink on paper and bound in contemporary quarter leather with marbled boards, was purchased by Yale Med from L. H. Butterfield (of Cambridge, Mass.) in January, 1963; no data available about place or date of production, but dealer states it came with papers of Prof. Francis Noyes Balch.
