Two paper labels pasted on front cover: a title-piece (heavily damaged) ending in "cum aliis"; a note with manuscript letter "Z". Manuscript table of contents on front pastedown. Ownership inscription on front flyleaf and back pastedown: Johannes Serlinger. Possible provenance inscription on p.3 (title 1): Conventus. Name of the convent rubbed away; Salzburg added in red modern pencil. Shelfmark "X. VI. 20" on upper pastedown. Accompanying documents include description by Taeuber and Weil, booksellers of Munich, from whom Dr. Harvey Cushing purchased the manuscript. Bookplate of Harvey Cushing on inside front cover. Hist bequest to Yale Medical School Library.
