To the sheriff of the county of [blank] Greeting Whereas the General Assembly of the state of Connecticut granted and laid a number of taxes, viz. [blank] on all the polls and rateable estates in said state, for public service: and whereas the part and proportion of said taxes arising upon the polls and rateable estates of the town of [blank] according to the lists thereof sent in to the Assembly, there still remains unpaid the sum of [blank] lawful money, as per account of the state treasurer, whereof execution remains to be done. And whereas [blank] was chosen and appointed by said town of [blank] to collect said rates or taxes ... but so far failed and neglected within the time limited by law to pay in said taxes to the said treasurer ... State of Connecticut. To the sheriff of the county of [blank] or his deputy, or either of the constables of the town of [blank] within said county, Greeting In the name of the governor and company of the state of Connecticut, you are hereby commanded to [blank] to appear before the [blank] County Court to be holden at [blank] within and for the county of [blank] on the [blank] Tuesday of [blank] then and there to answer unto [blank] in a plea, that to the plaintiff the defendant render the sum of [blank] which to the plaintiff, the [blank] justly owes by book, [blank] as by the [blank] book ready in court to be produced fully appears; which debt the defendant hath never paid [blank] (though often requested and demanded) [blank] which is to the damage of the plaintiff the sum of [blank] lawful money.
